Curmudgeonly GP Martin Ellingham's plans to resume his career as a consultant in London are put on hold with the birth of the baby he fathered with his now estranged partner, Louisa Glasson. At the surgery, new GP Dr Di Dibbs is excited to begin work, but Martin begins to have serious doubts about her competency after she misdiagnoses a patient and dishes out prescriptions for totally inappropriate drugs.

PC Joe Penhale has an unexpected visitor in the shape of his ex-wife Maggie, who appears to have completely forgotten that they split up a couple of years ago. Joe takes her to Large's restaurant, where fellow diner Martin quickly spots that Maggie is showing symptoms of amnesia. Louisa's mother Eleanor catches up with her old school friend Paul, a local fisherman, who takes her out on a date.
